Transition between phases, key stages, year groups and different educational settings can be a anxious time for parents/carers and their children, especially for children with SEN. At Easington Lane Primary School, staff work closely with parents, the children and outside agencies supporting the children to ensure a smooth transition.
For the children leaving in year 6, the SENCO and KS2 phase leader meet with staff in the KS3 provision to discuss individual needs and share information about each child. The year 6 children also attend transition days and if individuals require additional transition visits, the SENCO works with the various provisions to put a more personalised transition plan into place.
Any child with specific needs leaving the school is supported with transition into their new setting. Information is shared with SENCO of the school that the child is moving to and if the child has a Statement of Educational Needs or an Educational Health Care Plan the SENCO will offer to attend the initial transition meetings.
When children enter the Little Legs Nursery and School Nursery, the SENCO, Phase leader and staff work closely with parents, health visitors and professionals supporting children to share information to ensure appropriate provisions are in place for each child.
When moving to a new year group, all staff working with the child with SEN work together to support the transition into their new class.
